The Name of the book is, Classic Teaching Methods for Dance Teachers.
Paper back and spiral bound.
I found his old address but he has moved and copies seem to be out of print. If anyone has this book I would like to buy it.

I searched for it on worldcat and found 2 copies in libraries in NY and 1 in a library in TX, but that's it
