Ok, here is my take on this:
Point 1.
It takes a while to get used to dancing with small women, and women by nature seem to dance in a different way to men. sexist, possibly, but I think having breasts and wide spaced hips compared to knees (that's why teenage girls often have dodgy knees while they grow) can have an affect on the way a woman moves. It's not better or worse, it's different.
This, paired with unfamiliar music and a tiny bit of inertia (hey, we all have it I'm not saying it's anything wrong with you, it's just what happens when we dance with the same people all the time) may have meant it will take a few more attempts to "tune in" to each other. 4 years is not a long time. I don't care what people say, we are not dancing ceroc here. Tango is hard, and for followers the better you get the harder you realise tango is.
Point 2.
The UK with the exception of the insanely commercial tango scene in London is mostly not like the places other people have described.
I can think of about 5 couples/lone teachers outside of London for whom Tango is a full time and first strand income generator.
In general tango "teachers" in small towns and cities i.e. Cambridge, Norwich, Brighton etc "teach" because when they discovered tango, they wanted to bring it t the town they live in and in order to have more people to dance with, they had to start teaching people how.
The UK scene is mostly about bringing people into the fold. There would be no reason in this case for the teacher to make madahlia feel bad about her dancing to generate "business"-that is not what the scene is like. IME often "teachers" are quite happy to relinquish their "teaching" duties when someone else competent (not teaching dangerous things for example) turns up. It means they can finally relax and enjoy dancing with the tanguero/as they have grown.
Private lessons are not the norm here.

I wouldn't normally be. On this occasion I was so nervous about the dancing that I forgot about the music.

Agreed. This is why I think it was an issue with the leader.
I suppose you were too much under influence of dancing with a lady leader.
Or maybe sth else stressful was that day that got your concentration.
I know some ladies that lead, only one or two seems to be good leaders.
Also I went to some workshops that woman can lead, but it was only for workshop lessons.
I never saw them on the dance floor.
Some of good female leaders is Ariadna Naveira.

No, because you're honest and at the end of the class you'll say "ok, I did my best but you're still bad". By the way Fabian told this to me once, but it was in a group class, not a private.

I suspect that "inactive" means the account has been banned. Of course he'll make another account at some point with another dot added to the end, or some other foolish thing (until he get's banned again).
